Assessment and Diagnosis
Revision surgery of the neck (cervical spine) can be psychologically challenging for both patients and their loved ones. It is important to be assessed by a spine specialist to determine the possible causes of the previous surgery outcome.
Assessment may include imaging such as MRI with contrast, CT scans, and bone scans, as well as diagnostic injections to the nerve(s) or facet joint(s).
Revision Surgery Approach
In selected cases where a correctable cause is identified, revision surgery may be considered.
If a structural or surgical cause is identified, revision surgery may be performed with careful planning. Bone morphogenetic protein or demineralised bone matrix may be used to support fusion and bone healing.
Very often, fixation or stabilisation may need to be extended above and/or below the original surgical levels from the previous operation (Figure 1).

When Revision Surgery May Be Considered
Revision surgery may be considered when symptoms persist, worsen, or when there is evidence of non-healing, implant-related issues, or ongoing nerve compression, and evaluation by a spine surgeon helps determine the most appropriate revision strategy.
